Man checking his teeth.
If you've been told you have gum disease and are looking at your options, laser gum treatment probably came up either from your dentist or from your own research. It's increasingly common, it sounds high-tech, and it raises an obvious question: is it actually better than conventional treatment, or is it primarily a premium-priced version of something that works just as well the old way?
The honest answer is: for many patients, laser treatment genuinely adds value. For others, conventional scaling and root planing achieves the same result. Knowing which category you're in depends on the specifics of your gum disease — and that's a clinical assessment, not a decision you can make from an article.
What you can get from this article is enough understanding to have a productive conversation with Dr. Suchi Singh at Renew Dental Clinic in Sector 47, Noida about whether laser treatment is appropriate for your situation.
Gum disease starts as gingivitis — bacterial plaque at the gumline causes inflammation. The gum bleeds on brushing, looks redder than it should, and may be swollen. At this stage, it's reversible with professional cleaning and improved home hygiene.
Left unmanaged, gingivitis progresses to periodontitis. The infection extends below the gumline into the crevice between tooth and gum (the periodontal pocket). The body's immune response to the bacteria in these pockets causes bone destruction — the alveolar bone supporting the tooth is gradually resorbed. Pockets deepen. More root surface is exposed. Teeth eventually become mobile.
This bone loss is irreversible — treatment can halt it but not undo it. Which is why the goal of treatment is always to get to the infection as early as possible.
For established periodontitis, the standard treatment is scaling and root planing — often called "deep cleaning." It involves the dentist using ultrasonic scalers and hand curettes to remove calculus deposits from the root surfaces below the gumline. This is done under local anaesthesia because the subgingival (below-gum) area is sensitive.
Scaling and root planing works. Multiple systematic reviews confirm it reduces pocket depths, reduces bacterial counts, reduces bleeding on probing, and in many cases allows the gum tissue to reattach to the root surface as the infection resolves.
The limitation of conventional scaling is that it's a mechanical process — it removes what the instrument can physically reach. In very deep pockets (6 mm and above), or in pockets with complex anatomy, the instrument can't always adequately clean the root surface.
Dental lasers used for gum treatment — typically diode or Nd:YAG lasers — work by delivering controlled energy to the targeted tissue. In gum disease treatment, the laser does several things that conventional scaling doesn't.
It removes the diseased and infected gum tissue lining the pocket wall (the "pocket epithelium") more precisely than instruments can. This diseased tissue is one of the barriers to healing — removing it allows healthy tissue to grow in its place.
It kills bacteria within the pocket, including bacteria that have invaded the gum tissue itself. Conventional scaling removes bacteria on calculus and root surfaces but doesn't address the bacteria embedded in the soft tissue of the pocket wall. The laser's bactericidal effect extends into this tissue.
It causes less bleeding than conventional surgery by sealing small blood vessels as it works (laser coagulation). This means better visibility during treatment and faster healing of the soft tissue afterward.
It stimulates the remaining healthy tissue to regenerate — some laser protocols show evidence of stimulating new bone formation and connective tissue attachment in the post-treatment period, a result conventional scaling achieves less consistently.
The combined effect is deeper disinfection of the pocket, removal of the diseased lining, and potentially better regenerative outcomes, particularly in deeper pockets.
Laser gum treatment produces the most significant advantage over conventional scaling in patients with:
Deep periodontal pockets (5 mm and above). In shallower pockets, conventional scaling is highly effective. In deeper pockets where instrument access is compromised, the laser's ability to treat tissue beyond where instruments physically reach becomes more relevant.
Bleeding and inflamed gum tissue that hasn't responded adequately to conventional treatment. If scaling and root planing has been done but pockets remain deep and infection persists, laser treatment can address residual bacteria and diseased tissue more thoroughly.
Patients who are anxious about conventional gum surgery. Traditional periodontal surgery — flap surgery — involves cutting the gum, peeling it back, treating the underlying bone, and suturing it closed. Laser treatment, in many cases, achieves similar outcomes with significantly less invasiveness, less post-operative discomfort, and faster healing.
Patients who are medically compromised — on blood thinners, with uncontrolled diabetes, or at higher bleeding risk — benefit from the laser's haemostatic (bleeding-control) properties during treatment.
Patients with early to moderate gum disease (pockets of 3–4 mm, no significant bone loss) may achieve excellent results with conventional scaling without needing laser treatment. The additional benefit of laser in these cases is less pronounced.
At Renew Dental Clinic, laser gum treatment is preceded by a comprehensive periodontal assessment — pocket depths measured at six sites per tooth, bleeding on probing recorded, bone levels assessed on X-ray. This gives a complete baseline against which the treatment response can be measured.
Treatment is performed under local anaesthesia. The anaesthetised area won't feel the laser energy. The procedure involves the laser tip being guided gently along the pocket, treating the pocket wall and the root surface. There's minimal bleeding. No stitches in most cases.
Afterward: mild soreness for 1–3 days, managed with standard pain relief. The gum tissue settles and tightens over the weeks following treatment as healing occurs. A follow-up assessment 4–6 weeks later measures pocket depth reduction and any signs of gum reattachment.
Home hygiene becomes critically important after laser treatment. The laser has disinfected and removed diseased tissue, creating conditions for healing. Plaque allowed to re-accumulate will reinitiate infection in the same pockets.
Laser gum treatment — like all periodontal treatment — requires a maintenance schedule afterward. This isn't optional.
Periodontal bacteria recolonise treated pockets within months of treatment. Patients who return for 3–4 monthly professional cleaning and assessment appointments maintain their treatment results. Patients who attend for annual check-ups often present with recurrent disease despite successful initial treatment.
The maintenance schedule at Renew Dental is individualised — patients who've had significant periodontal disease and laser treatment are typically seen every 3 months for the first year, then adjusted based on how stable the condition remains.
Q: Is laser gum treatment painful?
A: During the procedure, no — it's done under local anaesthesia. Afterward, most patients find post-treatment discomfort milder than after conventional deep cleaning, partly because of the laser's tissue-sealing properties during treatment.
Q: How many sessions does laser gum treatment need?
A: Typically 2–4 sessions depending on the extent and severity of disease. The mouth is often treated in quadrants (one quarter at a time) to allow the patient to eat on untreated teeth during the treatment period.
Q: How long before I see results?
A: Gum tissue improvement becomes visible over 4–8 weeks as inflammation resolves and the pocket lining heals. A formal reassessment of pocket depths at 6 weeks gives an objective measure of response.
Q: Will my teeth feel different after the treatment?
A: Possibly. As inflammation resolves, gums may pull away slightly from where they were while swollen — meaning the gumline drops slightly and teeth may appear slightly longer. This isn't recession; it's the gum finding its healthy position after losing the inflammatory swelling. Some temporary sensitivity to cold may also occur as more root surface is exposed.
Q: Does laser gum treatment work permanently?
A: No treatment for gum disease is permanent in the sense of "one and done." The bacteria that cause periodontal disease live in every mouth. What laser treatment does is remove the infection and diseased tissue, creating conditions for healing. Maintenance — ongoing professional cleaning and excellent home hygiene — is what keeps it stable.
