A smile is usually called gummy when more than 3–4 mm of gum shows above the upper teeth.
A gummy smile — where more gum than feels proportionate is visible when smiling — affects a meaningful proportion of the population. Most people who have one became self-conscious about it in their teens and have been finding ways to smile with slightly less openness ever since. The good news is that it's genuinely treatable. The important caveat is that treatment needs to match the specific cause, because a gummy smile can result from several completely different anatomical situations — and what works for one doesn't necessarily work for another.
A smile is generally considered gummy when more than approximately 3–4 mm of gum tissue is visible above the upper teeth when smiling fully. This threshold is somewhat subjective — aesthetics are personal — but clinically, most practitioners use 3–4 mm as the point where the gum display significantly affects the smile's proportions.
The "ideal" smile shows approximately 75–100% of the upper front teeth and a small, consistent sliver of gum tissue at the top. The teeth appear as the visual centrepiece; the gum tissue frames them without dominating.
What creates excessive gum display varies considerably from patient to patient. This is the part most content about gummy smiles skips — it lists treatments without explaining that the right treatment depends entirely on which of the following applies.
Gingival hyperplasia (excess gum tissue): The gum tissue itself is more voluminous than normal, covering a greater proportion of the tooth crown. The teeth may be perfectly normal in length — they're just hidden under more gum than usual. Certain medications cause this (phenytoin for epilepsy, nifedipine for hypertension, cyclosporine for immune suppression), as can genetic predisposition.
In this case, the gum tissue is covering otherwise normal teeth. Removing the excess reveals teeth that were always there. This is the most directly treatable cause and usually responds excellently to laser gum contouring.
Short clinical crown / altered passive eruption: The teeth erupted into the mouth but the gum tissue didn't migrate to its final position during maturation — the gum remained high on the tooth crown, making teeth appear short even though they're actually full length inside the gum. This is extremely common and often mistaken for "small teeth" by patients who don't realise the teeth are full-length under the gum.
Crown lengthening (removing excess gum and sometimes a small amount of the underlying bone to establish a new gum level) exposes the full crown length. Results can be dramatic — teeth that appeared almost square suddenly have their natural length and proportion.
Hyperactive upper lip: The muscles controlling the upper lip's movement pull it higher than normal when smiling, exposing more gum than a normally-functioning lip would. The teeth and gum anatomy are entirely normal; the lip just moves too high.
This cause responds to Botox — a small injection into the lip elevator muscles (levator labii superioris) relaxes their pull, allowing the upper lip to rest lower during smiling. The effect is temporary (4–6 months) and needs repeating.
Vertical maxillary excess (VME): The jawbone itself has grown longer than proportionate — the upper jaw is too long vertically. This is the most significant structural cause and the one that produces the most pronounced gummy smile. When the jaw is the cause, the gum tissue is normal, the teeth are normal, the lip is normal — but the entire upper jaw is longer than it should be relative to facial proportions.
VME is the only cause that requires orthognathic (jaw) surgery for definitive correction. The other treatments above provide minimal improvement when the jaw length is the primary driver.
Combination causes: Many patients have more than one contributing factor. A slight gingival hyperplasia plus some altered passive eruption plus mildly elevated lip mobility — the gummy smile results from the combination, and treatment needs to address all relevant components.
Laser gum contouring is the most commonly performed gummy smile treatment in Noida for appropriate cases. A dental laser is used to precisely remove and reshape excess gum tissue, establishing a new, lower gumline that exposes more of the tooth crown.
Advantages of the laser approach: minimal bleeding (the laser seals blood vessels as it cuts), very precise tissue removal, significantly faster healing than conventional scalpel surgery, less post-procedure discomfort. Local anaesthesia is used for comfort during the procedure.
The procedure takes 30–60 minutes. Results are immediately visible — the gumline is reshaped and more tooth is visible before the patient leaves the chair. There is some initial swelling and the gum tissue needs 2–3 weeks to fully mature and settle. The final result is stable and permanent.
Laser contouring is appropriate for: excess gum tissue (gingival hyperplasia) and altered passive eruption (where the gum hasn't migrated to its natural position). It's not appropriate when the cause is lip mobility or jaw length — in those cases, contouring the gum doesn't address the reason for the excessive display.
Crown lengthening — when bone removal is also needed alongside gum reshaping — is a slightly more involved surgical procedure. If the gum sits high because both gum tissue and bone are positioned more coronally than they should be, removing gum tissue alone doesn't produce a stable result. The bone level determines where the gum will eventually settle; without addressing it, the gum tissue grows back. Crown lengthening addresses both the gum and the bone level, producing a stable, permanent result.
Botox for hyperactive lip addresses the specific cause of lip-mobility-driven gummy smiles without touching the gum or teeth. The result is temporary but produces a very natural-looking improvement in gummy smile patients whose primary issue is how high the lip travels. Many patients prefer repeating Botox over committing to a surgical procedure.
At Renew Dental Clinic in Sector 47, Noida, Dr. Suchi Singh assesses which cause is driving the gummy smile before recommending treatment. Recommending laser contouring for a patient whose gummy smile comes from a hyperactive lip produces a disappointing result because the cause was never addressed. The assessment comes first.
For altered passive eruption treated with laser contouring or crown lengthening: the transformation is often striking. Teeth that appeared short and square suddenly have their natural proportions — often considerably longer-looking without any other change. The gumline becomes more regular and even across the front teeth.
For gingival hyperplasia: removal of the excess tissue reveals teeth in their correct proportion. The smile looks balanced — more tooth, less gum.
For Botox: the upper lip's resting position when smiling changes so that less gum is visible. The teeth themselves look the same; the frame around them changes.
Q: Is laser gum contouring painful? A: During the procedure, no — local anaesthesia is used. Afterward, mild soreness for 3–5 days, managed with standard pain relief. Significantly more comfortable than the equivalent scalpel procedure.
Q: How long before I can see the final result? A: Immediately after the procedure, you can see the new gumline. The tissue takes 2–3 weeks to fully mature. Final assessment is done 4–6 weeks post-procedure.
Q: Will the gum grow back after laser treatment? A: Not in well-planned cases where the biological width is respected — meaning the bone level is correctly accounted for. If only gum is removed without bone reshaping in cases where bone adjustment was necessary, the gum can partially regrow. This is why the assessment determines whether gum-only treatment is sufficient or whether bone needs addressing.
Q: Is Botox for gummy smile safe? Does it affect smiling normally? A: Yes, safe when performed by an experienced practitioner with appropriate dosing. The lip is relaxed but not paralysed — smiling is normal, just with the lip travelling slightly less high. Asymmetry from uneven dosing is the main technical risk, resolved by proper placement and occasional minor dose adjustment at a follow-up.
